Waiver for In-Person Self-Defense Class

To submit your waiver, please follow these steps:

1. Click here to read the Participation Waiver & Media Release in a new tab
   
   
2. Return to this page & click “Add to Cart”
   
   
3. On the checkout page check the box confirming you have read and agreed to the waiver
   
   
4. Click the “Checkout” button.
   
   
5. Enter your contact information: full name, email address, and billing address
   
   
6. Click the "Complete Order" button
   
   
7. No payment or credit card is required — this process simply records your signed waiver in our system.
   
   Note: A billing address is required by the system, but no payment is asked for or collected and the info is not used beyond registration.
